SERVICES

ROUTINE CARE

Hospice Staff will provide regularly scheduled visits to a patient’s place of residence to adequately manage symptoms and meet the needs of the patient and family.

CONTINUOUS CARE

When death is imminent and the primary caregiver is unable to cope, hospice provides 24 hours care for short-term crisis management.

Hospis care

INPATIENT CARE

When a patient’s symptom cannot be managed at home, the family has the option to have their needs met at Medicare Certified facilities mostly Skilled Nursing and Convalescent Homes. Room and board have covered this level of care. Impatient care is usually short-term.

RESPITE CARE

When family members, caregivers need to take a break from the day-to-day care of the patient, hospice offers a 5-day respite relief. Hospice will transport the patient to a contracted facility where room and board are covered for a maximum of 5 days.

WHO QUALIFIES FOR HOSPICE?

There are three criteria for hospice eligibility:

  1. The patient has been diagnosed by a physician as having
    an advanced disease with a life-limited expectancy.
  2. Curative measures have ended and the focus is now on
    physical, emotional, and spiritual comfort.
  3. The patient’s physician authorizes hospice care.

WHO PAYS FOR HOSPICE CARE?

Comfort Choice Hospice, Inc. is reimbursed by Medicare, MediCal, commercial health insurance plans including HMOs and managed care organizations.

WHAT DOES THE FAMILY PROVIDE?

  • Primary caregivers for patients 24/7
  • A place of care and costs for home, nursing home,
    assisted living, or Board and Care facility.
  • Bedding, clothing, and personal care items.
